Michael Cook

MICHAEL E. COOK, 51, died at 3:58 p.m. April 1, 2005, at St. Rita's Medical Center. He was born December 20, 1953, in Lima, Ohio to Howard A. and Alice Craig Cook, who preceded him in death. On September 28, 1975, in Lima, Ohio, he married Carolyn Whittaker, who survives in Elida, Ohio. Mr. Cook worked for St. Rita's Medical Center as a maintenance coordinator. He was a trustee at Grace Baptist Church for many years. He shared the gift of life through donation. Survivors also include four children, Steven Cook, David Cook and Deborah Cook, all of Columbus, Ohio, and Jason Cook of Elida, Ohio; two sisters, Marcene Cook and Kathleen Cook, both of Lima, Ohio; and a brother, Howard E. Cook of Virginia Beach, Virginia. He was preceded in death by a brother, Shelby Cook; and a sister, Eileen Cook. Services will begin at noon Tuesday at Grace Baptist Church. Pastor Matthew Oliver will officiate. Burial will be in Ward Cemetery, Lima, Ohio. Friends may call from 2 to 4 and 6 to 8 p.m. today at Chamberlain-Huckeriede Funeral Home, Lima, Ohio.
